Confirmed Booking Form
Thank you so much for inviting us to be a part of your event. Please complete the details below to help us create your booking confirmation / invoice. By completing this form you commit to the payment and booking terms as detailed below.
If you have a preferred date for your trial, please let us know, however please know that we do keep all trial appointments tentative to allow for event bookings.
Deposits are required to secure all bookings and details of your deposit amount will be on the invoice.

PAYMENT TERMS & CONDITIONS
Balance payments are due at least 3 days prior to the event.
Deposits must be paid by the due date. Your event date will only be held until the deposit due date or your reservation may be released to another client.

YOUR BOOKING COMMITMENT
The total of this invoice will be the minimum amount required on the day of service. No refund or discount will be given for services not rendered on the day. Full payment will be owing, regardless of your cancellations, from 10 days prior to the event unless otherwise arranged (14 days prior on Melbourne Cup Long Weekend).
You commit to providing us with a smoke-free environment in which to work. You agree that a stylist may choose to leave prior to services being rendered if a smoke free environment is not available, however you will still be charged the invoice total.
We do our utmost to arrive & finish at the times indicated and agreed. You agree to ensure that you and your party are available when we arrive & throughout our booking time. If your party is not available you understand that we may have to leave at the designated finish time before completing the full invoiced service on which you will be charged.
